2. Curriculum Checklist of Evaluation
3. NMAT Original (For MEDICINE)
4. TOR Original
5. Clership Certificate (MED)/Internship/Field Study
6. Birth Certificate/Foreign Passport
7. Copy of Previous school TOR for Transfer students.
8. Copy of Comprehensive Examination (Full Payment)
9. Approved letter request for the president in available. (form 1)
10. Graduation Fee/Journal Fee/Alumni Fee (Submitted All the Required Documents)
